29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Shipping area, Wyman Gordon, 244 Worcester Street, North Grafton, Massachusetts; The microwave used by employees in the shipping area was plugged into a relocatable power tap (RPT). The RPT had a two prong adapter that defeated the grounding of the RPT which exposed employees to an electric shock hazard while using the microwave oven.
