29 CFR 1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service. a. On or about June 9, 2015, located in the paint area of the facility, there was a nylon sling that showed significant signs of wear and damage that had not been removed from service, thereby exposing employees to a struck-by hazard. b. On or about June 9, 2015, located in the Stair Bay, there was a nylon sling that showed significant signs of wear and damage that had not been removed from services, thereby exposing employees to a struck-by hazard. c. On or about June 9, 2015, located in the Stair Bay, there was a nylon sling that showed significant signs of wear and damage that was being used to lower a steel I-beam structure by an employee while the nylon strap was on the sharp edges of the I-beam . An inspection of the sling before and during use had not been performed, exposing the employee to a stuck-by hazard.
