29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. Wilbert, Inc. - Bellevue, OH: On or about May 14, 2015, the employer did not ensure that lockout devices were affixed to the robot associated with press A56 before employees changed the press mold. A potential for unexpected startup of the robot existed while employees performed mold changing operations.
