29 CFR 1910.333(b)(2)(ii)(B): The circuits and equipment to be worked on were not disconnected from all electric energy sources: a) G Furnace - The employer did not ensure that the Electron Beam gun was disconnected from all electric energy sources. The main 480 volt breaker MDP-2A was in the closed (on) position and the downstream breaker MCC-2 was locked in the closed (on) position. Employees were exposed to possible electrocution when working on or near the electron beam guns, on or about March 28, 2015. IN THE ALTERNATIVE: 29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources. a) G Furnace - The employer did not ensure that the Electron Beam gun was disconnected from all electric energy sources. The main 480 volt breaker MDP-2A was in the closed (on) position and the downstream breaker MCC-2 was locked in the closed (on) position. Employees were exposed to possible electrocution when working on or near the electron beam guns, on or about March 28, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
