29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Production Area - Approximately fifteen employees who operated the facilitys powered industrial trucks including, but not limited to two Caterpillar propane sit-down forklifts (model T120C, serial number 55L665 and model T60B), and two electric stand-up forklifts (one Raymond model 21R30TT, serial number 021.85.01050 and one Deka model LT 150-20, serial number 38106) were not trained as required by the standard. On or about 6/27/13. Abatement Note: Forklift operator training consists of a combination of formal instruction (e.g., lecture, discussion, interactive computer based training, video tape, written material, etc.), practical training (demonstration performed by the trainer and practical exercises performed by the trainee), and an evaluation of the operators performance in the workplace. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
