29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process: At the establishment, the employer did not perform a management of change for the modification of the ammonia refrigeration unit in the two engine rooms approximately 12 years ago. The employer did not implement the procedure to manage the changes of installation and intention to use of the emergency shutdown control switches located on the outside of the engine rooms. Employees were exposed to catastrophic release of ammonia which result in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
