29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information. a)On or about October 4, 2010 and at times prior thereto, the written operating procedures had not been implemented and were not being used by operators. b)On or about October 4, 2010 and at times prior thereto, the operating procedure for oil pot draining was not clear in that it listed numbered valves in sequence that were not labeled on the oil pot valves. Note: Abatement certification and supporting documentation are required for this item.
