29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: On or about August 28, 2023, at the Sandage 1 Pump Station, Employee #1 was assigned to work in trenches to perform tasks including, but not limited to pipe installation. Employee #1 was not trained to recognize the hazards associated with trenching and excavation and the methods to control and eliminate the hazards.
