29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. (a) Debone - On or about July 15, 2023, the employer did not ensure sanitation employees affixed a locking device to the electrical disconnect switch for the Transfer Conveyor when a sanitation employee was standing and squatting on the conveyor to perform hand scrubbing sanitation work exposing the employee to a caught-in hazard.
