29 CFR 1910.218(f)(1)(i): The power to mechanical forging press(es) was not locked out when the die(s) were being changed or maintenance was being performed on the press(es). (a) Production Floor/Press Area: On or about June 2, 2023, and times prior, the Ajax 1300-ton forging press was not locked out during die bolt tightening and die residue cleaning tasks, exposing employees to amputation hazards. (b) Production Floor/Press Area: On or about May 18, 2023, and times prior, the Erie 1600-ton forging press was not locked out during die bolt tightening and die residue cleaning tasks, exposing employees to amputation hazards.
