29 CFR 1910.119(j)(4)(iv): The documentation of each inspection and test that was performed on the process did not identify the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test: At a worksite located at 1645 NW 550 Road, Kingsville, MO 64061: Employees are exposed to fire and explosion hazards. This was demonstrated on or about January 27, 2022, and at times prior to when employees were working with energetic materials at a small arms ammunition manufacturing facility. The employer did not document the inspection and testing that was performed for equipment, instruments, or other components in the covered process; including but not limited to, heat sealer, plate loading, crimp press, hand stacking, Waterbury, 20mm staking, and V&O.
