29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces are permit-required confined spaces. Central Paper Stock, Inc. located at 6665 Jonas Place in Berkeley, MO 63134: The employer is failing to assess the workplace for permit-required confined spaces. On or about 10/26/2021, employees were required to enter spaces with internal configurations which could entrap the entrant and posed serious safety hazards during baler or grinder operation. The employer failed to assess the following confined spaces: a) The large "Harris Selco" baler (Model: HLO-8221AR-150; Serial No: 39770079); b) The small "Harris Selco" baler (Model:L125-2-10/8 ; Serial No:08138825 ); c) The "Retech" book grinder (Model: RG 52; Serial No: 7739101); and, d) The plastic "Badger" baler (Model: BADGER L-100S-2-10/7; Serial No: 8658).
