29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): The employer is failing to protect employees from caught-in and amputation hazards during foam filling operations. This was most recently documented on or about 05/31/2019 at the Shamrock Tire & Auto Service, Inc. dba Tire Dealers Wholesale facility located at 3003 S. Broadway St. Wichita, Kansas 67216. The employer did not provide guards around the pulleys for the auger which were located approx. 72 inches off the floor.
