29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions. On or about October 22, 2018, and at times prior there to, in the old break room building, electrical outlet boxes used to energize portable equipment wiring was not protected from abrasions when ran into metal knock-outs, exposing employees to electrical shock hazards: a) On the north wall, two (2) electrical outlets without wire protection; and b) On the south wall, two (2) electrical outlets without wiring protection.
