Text For Citation: 01 Item/Group: 008A Hazard:
29 CFR 1910.146(e)(1): Before entry is authorized, the employer shall document the completion of measures required by paragraph (d)(3) of this section by preparing an entry permit. a. On or about May 25, 2018, an employee cleaning the inside of a semi-truck tanker was pronounced dead following a confined space rescue. The employee was found to have suffered chemical asphyxia by vitiated atmosphere, hydrogen sulfide and hydrogen cyanide gases. Where the hazardous atmosphere had not been eliminated prior to entry a permit-required confined space permit had not been completed.
