29 CFR 1915.113(b)(1): Hooks without a known safe working load were not tested to twice the intended safe working load before they were initially put into use: On February 7, 2018, Slip, Yard 1, Employees were exposed to struck-by hazards while removing the anchor chain from the USS Harkness, using a non-tested hook with an unknown safe work load, prior to initial use of the hook.
