29 CFR 1910.269(a)(2)(i)(A): Each employee was not trained in, and familiar with, the safety-related work practices, safety procedures, and other safety requirements in this section that pertain to his or her job assignments. Substation- Employees entering the substation and engaged in work activities including, but not limited to, thawing out pneumatic air lines controlling the closing of circuit breakers with open flames were not protected from electric shock and arc flash hazards where the employer did not train employees in regards to the minimum approach distances to exposed energized parts in the substation, required personal protective equipment to be worn, and the use of conductive tools and equipment within the substation.
