29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about 6/2/2016 in the oil lube shop at 2434 West Broad Street in Athens, GA: Electrical switch at up to 120 volts was connected to a knockout metal outlet box receptacle that was not securely and rigidly fastened to a support structure or otherwise secured. The employer failed to secure the box and prevent the employees from being exposed to and electrical hazard.
