29 CFR 1910.103(b)(1)(iv)(c): Storage containers, piping, valves, regulating equipment, and other accessories were not readily accessible, and shall be protected against physical damage and against tampering: A.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA, 15824, loading dock ramp, on or about November 23, 2015 and times prior thereto: Items including, but not limited to, plastic strapping / banding, were hanging from the hydrogen quarter-turn emergency shut-off valve. In addition, items including, but not limited to, boxes and drums were stored in front of the hydrogen quarter-turn emergency shut-off valve.
