29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: A.) Warren Railcar Service, Inc. - On or about, 8/19/15, and at times prior thereto, the company was utilizing a man basket forklift attachment on the Yale forklift (serial #P-3059516). The manufacturer's approval to use the attachment with the forklift was not obtained. In addition, associated data and capacity plates were not changed to reflect the attachment.
