29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about July 8, 2014, employees were exposed to caught in and struck by hazards in that the employer used an unapproved man basket to lift employees by use of fork lift to change lights in garage area and perform maintenance on the crane.
