29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a. On or about September 5, 2013, at Line No. 2 of the production area of the above addressed worksite, a fan (i.e. Dayton, Model 3C21BG) lacked a ground prong on its attachment plug, exposing employees to an electrical hazard. b. On or about September 17, 2013, at Line No. 4 of the production area of the above addressed worksite, a fan (i.e. Model FE-70D3TFOT, Serial No. 001769) lacked a ground prong on its attachment plug, exposing employees to an electrical hazard.
