29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Shop Area; On or about April 1, 2013 and at times prior, employees were exposed to the caught in hazards created by the unguarded Monarch Lathe. b) Shop Area; On or about April 1, 2013 and at times prior, employees were exposed to the caught in hazards created by the unguarded Ta Shing Lathe. c) Shop Area; On or about April 1, 2013 and at times prior, employees were exposed to the caught in hazards created by the unguarded Summit Lathe. d) Shop Area; On or about April 1, 2013 and at times prior, employees were exposed to the caught in hazards created by the unguarded Lodge & Shipley Lathe. e) Shop Area; On or about April 1, 2013 and at times prior, employees were exposed to the caught in hazards created by the unguarded JET 134 OGHE Lathe. g) Shop Area; On or about April 1, 2013 and at times prior, employees were exposed to the flying chips and sparks created by an unguarded JET Mill. h) Shop Area; On or about April 1, 2013 and at times prior, employees were exposed to the caught in hazards created by the unguarded bench wire wheel. i) Shop Area; On or about April 1, 2013 and at times prior, employees were exposed to the caught in hazard created by the unguarded Hydraulic Cylinder Hone.
