29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation no longer existed: LOCATION: ICE MACHINES: The employer did not ensure that verification of liquid anhydrous ammonia isolation was continued where the possibility of re-accumulation of hazardous levels of ammonia vapors were possible, until after screen cleaning or other maintenace was completed.
