29 CFR 1910.119(f)(3) The employer did not certify annually that the operating procedures for the Hydrocracker Unit were current and accurate. This standard was violated on or about April 19, 2011 when the employer failed to certify the Hydrocracker Unit operating procedures were current and accurate. The SSEP procedure manual had been certified on April 19, 2010 and should have been certified again on or before that date in 2011. However, the actual date in 2011 that the procedures were certified was April 29, ten days later than it should have been.
