29 CFR 1910.119(e)(6): The employer did not ensure after the initial process haz ard analysis that the process hazard analysis was updated and revalidated at lea st every five (5) years by a team meeting the requirements of 29 CFR 1910.119(e) (4) to assure that the process hazard analysis was consistent with the current p rocess: The employer does not ensure that the process hazard analysis is consist ent with the current process: a) This violation occurred on or about October 13, 2010 in the Butadiene Recovery Area of Lion Copolymer - Baton Rouge, when Lion Copolymer failed to conduct a re validation of the BD Recovery PHA since October 4, 2005. This exposed employees to fire and explosion hazards. Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of this citation, the employer must submit documentation showi ng that it is in compliance with the standard, including describing the steps it is taking to revalidate PHA's.
