29 CFR 1910.119(d)(3)(ii): The employer did not document that the equipment in the process complied with recognized and generally accepted good engineering practices (RAGAGEP): The employer could not document in their PSI that equipment in the process complied with RAGAGEP in the following cases: b- Hess Port Reading Refinery, Gas Con - The employer did not comply with RAGAGEP such as but not limited to ASME Boiler and Pressure Vessel Code, Division 1 (BPVC), Section VIII, UG-135(d) when it failed to provide adequate controls to ensure that the outlet block valve for RV-D-603-2 and a 6 inch block valve from E-641A C3/C4 Splitter Feed/Bottoms Exchanger to T- 621 C3/C4 Splitter were chain locked open. Conditions noted on or about 12/18/08 and 02/09/09, respectively. c- Hess Port Reading Refinery, FCCU - The employer did not comply RAGAGEP such as but not limited to ASME Boiler and Pressure Vessel Code, Division 1 (BPVC), Section VIII, UG-135(d) when it failed to provide adequate controls to ensure that the inlet block valve for RV-D-514-3 High Pressure Disengaging Drum was chain locked open. Condition noted on or about 12/16/08. d- Hess Port Reading Refinery, Alky - The employer did not comply RAGAGEP such as but not limited to ASME Boiler and Pressure Vessel Code, Division 1 (BPVC), Section VIII, UG-135(d) when it failed to provide adequate controls to ensure that the inlet valve and shell side block valve of E- 711B Feed/Effluent Exchanger were chain locked open. Condition noted on or about 02/27/09. f- Hess Port Reading Refinery, Dimersol - The employer did not comply with RAGAGEP such as but not limited to API 576 Section 4.9.2 when it failed to install a tell-tale indicator between the RD and pressure RV for S-2204B Caustic Rag Layer Filter. Condition noted on or about 03/05/09.
