29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:(a) (LOCATION) (IDENTIFY EQUIPMENT AND CONDITION(S)) (STATE HAZARD(S) TO EMPLOYEE(S)) On or about February 25, 2025, and before at the worksite located at 34 Kelly Road, Buffalo, MO 65622 employees were exposed to electrocution hazards while conducting grain operating activities. Employees were operating buttons and switches in the control room next to dump shed/truck unloading area that were located next to an electrical fixture/box with a missing cover and exposed wires.
