29 CFR 1910.272(g)(1)(i): The employer did not issue a permit for entering bins, silos or tanks in the grain handling facility before the employees entered the bins, silos, or tanks: At the worksite located at 34 Kelly Road, Buffalo, MO 65622, employees were exposed but not limited to caught in, fall, suffocation, fire, and explosion hazards while conducting feed mill and grain handling operations. The employer did not issue a bin entry permit prior to employees entering into any grain bin. Instances of exposure include but are not limited to: 1. On or about January 13, 2025, employees entered bin number 2 to clean out the wheat from the bin. 2. On February 24, 2025, an employee was operating the sweep auger on bin number 4 to clean out the corn. The employee then entered the bin.
