29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles and/or altered to allow the grounding pole of a plug to be inserted into slots intended for connection-carrying conductors. On or about March 13, 2024, and at times prior thereto, employees were exposed to an electrical hazard due to the grounding conductor being bypassed.
