29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees. On or about May 5, 2023, at the Shell Chemical facility located in Deer Park Texas, employees were exposed to fire and chemicals release hazards when the employer did train employees effectively on the function of the energy control programs for the following but not limited to: a.) A chain and lock on the actuator handwheel of the MOV-2516 (Motorized Operated Valve) were removed without following energy isolation procedures. b.) Electrical energy isolations for MOV-2514 and MOV-2516 were not shown to craft leads and were not locked. c.) Permit verifiers did not review and ensure energy isolation list (EIL) and energy isolation drawing were matched and electrical energy isolations for MOV-2514 and MOV-2516 were locked.
