29 CFR 1910.147(c)(5)(ii)(C)(1): Lockout devices were not substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools: (a) Mechanical Room; On November 18, 2022, the employer did not ensure employee(s) performing service and maintenance of the Trane and York Chillers in the mechanical room were provided equipment to identify and isolate energy sources, exposing employees to struck by and electrical hazards. (b) Mechanical room; On November 18, 2022, employees performing maintenance on the electrical panel utilized plastic ties that were not substantial enough to prevent removal without the use of excessive force or unusual techniques to lock out sources of hazardous energy, exposing employees to electrical hazards.
