29 CFR 1910.265(d)(1)(ii)(b): The lifting cylinders of all hydraulically operated log handling machines was not equipped with a positive device for preventing the uncontrolled lowering of the load or forks in case of a failure in the hydraulic system: (a) On or about November 2, 2021, and at times prior thereto at the Cooper Scragg mill, the log loader had visible hydraulic leaks from the hoses and cylinders exposing employees to struck-by and crushed-by hazards.
