29 CFR 1910.178(l)(6): The employer did not certify that each powered industrial truck operator has been trained and evaluated, as required by paragraph (l) with a certification that includes the name of the operator, the date of the training, the date of the evaluation, and the identity of the person performing the training and evaluation: a) Throughout the warehouse, on or about July 1, 2021, the employer failed to ensure employees operating powered industrial trucks were certified through training and evaluation, thereby exposing employees to struck-by and other hazards associated with the improper use of powered industrial trucks.
