29 CFR 1910.119(l)(1): Procedures to manage changes to process chemicals, technology, equipment and procedures, and changes to facilities that affect a covered process were not implemented: (a) Location - 330 Ballardvale Street, Wilmington, MA: On or about May 27, 2021, the facility's emergency action plan was changed to include a shelter in place procedure in response to releases of ammonia from the refrigeration system and management of change procedures established by the employer were not followed. (b) Location - 330 Ballardvale Street, Wilmington, MA: On or about May 27, 2021, the facility's emergency action plan was changed to prohibit the employees of the facility from assisting in responses to releases of ammonia from the refrigeration system and management of change procedures established by the employer were not followed. (c) Location - 330 Ballardvale Street Wilmington, MA: On or about May 27, 2021, the facility's Environmental, Health, and Safety Manager left the company and management of change procedures established by the employer were not followed. (d) Location - 330 Ballardvale Street, Wilmington, MA: On or about May 27, 2021, a compressor was taken offline with no intention of returning it to service and management of change procedures established by the employer were not followed.
