29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a) Building Exterior front: Employees were using pipe scaffold frames and cross braces to access the scaffold, exposing employees to a fall of approximately 20 feet to the ground surface, on or about 3/3/21. b) Building Exterior front: Employees were using an extension ladder that was about 3 feet away from the pipe scaffold to access the working platform, exposing employee to a fall hazard, on or about 3/3/21.
