29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: Stunning room: On or about August 29, 2019, and at times prior thereto, employees were exposed to an electrical hazard, while using a Hubert Haas Stunning Device with the outer insulation pulled away at the plug which did not provide strain relief.
