29 CFR 1910.147(c)(5)(ii)(D): Lockout/ tagout devices did not indicate the identity of the employee applying the device(s): The employer does not ensure tagout devices are identifiable by employee names, this violation was observed on or about July 25, 2019, in the log ride control room where two lockout devices were applied to a disconnect panel that did not identify the employee applying the devices, exposing employees to electrical and struck-by hazards. Pursuant to 29 CFR 1903.19, within (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure lockout / tagout devices indicate the identity of the employee applying the device(s).
