29 CFR 1915.159(c)(5): Defective fall arrest system components were not removed from service: A. Near Dry Dock 1: On, or about, October 19, 2018, a Miller harness provided for use on the Genie Z-60/34 aerial lift was damaged in that he webbing material was cut. B. Small Dry Dock: On, or about, October 19, 2018, the self-closing carabiner provided on the Miller TurboLite self-retracting lanyard was stuck in the unlocked position as it was attached to the anchor point on the Marklift Challenger aerial lift work platform.
