29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. a. On or about April 9, 2018, and at times thereto, employees were exposed to an electrical shock and/or arc flash hazard due to 480 volt conductors exposed to the sharp edge of a flexible conduit that was pulled out of the connector fitting entering the bottom of the disconnect switch feeding the outfeed conveyor motor. b. On or about April 9, 2018, and at times thereto, employees were exposed to an electrical shock and/or arc flash hazard due to 480 volt conductors exposed to the sharp edges of a flexible conduit that was damaged and separated at the outfeed conveyor motor. c. On or about April 9, 2018, and at times thereto, employees were exposed to an electrical shock and/or arc flash hazard due to 480 volt conductors exposed to the sharp edges of a flexible conduit that was damaged and separated at the magnetic conveyor motor.
