29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the required training consisting of a combination of formal instruction , practical training , and evaluation of the operator's performance in the workplace: a) Atlantic Precast Concrete - On or about March 15, 2018, an employee was operating a 20,000 pound capacity forklift without completing the formal instruction and had not been evaluated for forklift certification, exposing the employee or surrounding employees to forklift hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
