29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: On or about May 10, 2018, employees working in the Press Area were exposed to electrical shock hazards because: a. the computer and monitor located behind the Cincinnati Press Brake, Machine Number M833, were plugged into a 6-outlet power strip which was plugged into an extension cord which was plugged into another 6-outlet power strip which was plugged into a wall outlet. b. the two fans located between Cincinnati Press Brake, Machine Number M833, and Cincinnati Press Brake, Machine Number 1179, were each plugged into a 4-outlet electrical adapter which was plugged into an extension cord which was plugged into a 6-outlet power strip which was plugged into a wall outlet. c. the pedestal fan located between Cincinnati Press Brake, Machine Number 1179, and Cincinnati Press Brake, Machine Number 1167, was plugged into an extension cord which was plugged into another extension cord which was plugged into a wall outlet. d. the fluorescent light fixture located above the Amada America, Inc. Press Brake, Machine Number 1164, was plugged into an extension cord which was plugged into a wall outlet.
