29 CFR 1910.119(j)(5): Employer did not correct deficiencies in equipment that are outside acceptable limits before further use or in a safe and timely manner. In The Alternative 29 FR 1910.225(b): Employer did not ensure machines designed for a fixed location were securely anchored to prevent walking or moving. This violation was most recently observed on or prior to December 6, 2017 in the Benzoyl Peroxide (BPO) Unit at United Initiators located in Helena, Arkansas when the employer failed to correct structural equipment deficiencies; including but not limited to corroded and unsecured supports for: Instance a: The CF-1101 and CF-1102 supports exhibited severe corrosion and were unsecured. Instance b: The E-1102 and E-1103 supports exhibited severe corrosion and were unsecured.
