29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a. On or about August 17, 2017, located at Table 4, Roll 4-21, the company used a checklist for employees to perform lockout/tagout activities that did not include a specific statement of intended use, specific procedural steps for shutting down, all steps of isolating the energy isolating devices and the order of isolating. The checklist also did not include specific procedural steps for the placement, removal and transfer of lockout devices and how to verify the effectiveness of the lockout devices, thereby exposing the employees to the hazards associated with performing lockout activities on the Table 4 rollers.
