29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about June 14, 2017, at the Art Street plant, operators are required to do such task as, clean out the machine, unjam machines, and change saw blades and drill bits on machines. Such machines are, but not limited to: a) On or about June 14, 2017, near detail aisle the Ficep machine was being worked on and was not locked out. Employees exposed to the hazard of being caught-in the machine from unexpected start-up. The machine contains such energies as electrical, hydraulic and pneumatic. b) On or about June 14, 2017, in detail aisle, the Kinetic plasma cutter 3000 and Kinetic plasma cutter K5000xmc are cleaned out by the operators and are not locked out. Employees exposed to unexpected start-up of the machine. The plasma cutters contain such energies as electrical, thermal and gas. c) On or about June 14, 2017, at the beamline area the HE&M Saw, the operators will cut I beams. The operators will change the blades on the machine and will clean out the metal shavings and does not lockout the machine. Employees exposed to a struck-by or laceration hard from unexpected start-up of the machine and the saw arm falling. The HE&M saw contains such energies as electrical, hydraulic and gravitation (saw arm falling). d) On or about June 14, 2017, at the beamline area the Control Automation drill is used to drill holes in the I beams. There are three drill bits that get changed by the operator. The operator climbs into the machine to change the middle drill bit and does not lockout the machine. The control Automation drill contains such energies as electrical and hydraulic. Employees exposed to a caught-in hazard from unexpected start-up of the machine.
