29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about April 20th, employees were exposed to electric shock hazards due to improper electrical installations in the following areas: a. At Garment Mend, the Thermoset machine was plugged into a relocatable power tap which was powered by a portable metal outlet box connected with flexible cord; b. At the Shirt-Pant Press Area, there was a metal outlet box intended for wall mounting attached to an extension cord and used to supply 110-volt service to a floor fan.
