29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On or about January 11, 2017: a. On the north end of the mill where employees were exposed to electrical and fire hazards while working near an electrical box that powered the chipper shaker that had a knock out hole opening at the top of the box without that opening being effectively closed. b. On the east side of the mill where employees were exposed to electrical and fire hazards while working near an electrical box that contained the Tyrone brand feed works soft start control without an unused opening at the bottom of the electrical box being effectively closed. c. On the south side of the mill where employees were exposed to electrical and fire hazards while working near the head rig saw electrical disconnect that had a knock out hole opening on the side of the box next to the disconnect switch without the unused opening being effectively closed. d. On the south side of the mill where employees were exposed to electrical and fire hazards while working near the head rig saw conveyor table electrical box that had a knock out hole opening on the top of the box without the unused opening being effectively closed.
