29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. On or about February 8, 2017, in the maintenance shop, an employee operated the Hyd-mech horizontal band saw, Model 8-20A, Serial Number 81205785FD, that had unused portions of the blade not guarded, thereby exposing the employee to an amputation hazard. b. On or about February 7, 2017, in the maintenance shop, an employee operated the Marvel Series 8 Mark 1 vertical band saw, Serial Number 821613-W, that had unused portions of the blade below the saw table not guarded, thereby exposing the employee to an amputation hazard. c. On or about February 7, 2017, in the maintenance shop, an employee operated the Marvel Series 8 Mark 1 vertical band saw, Serial Number 821613-W, that had the unused rear blade above the saw table not adequately guarded, thereby exposing the employee to an amputation hazard. d. On or about February 7, 2017, in the maintenance shop, an employee operated an Atrump vertical manual mill, Model K6F, Serial Number K6F-153, that did not have a chip guard to prevent contact with the flying chips, exposing the employee to a struck-by hazard. e. On or about February 7, 2017, in the maintenance shop, an employee operated a LeBlond 24 inch regal lathe that did not have a guard to prevent contact with the rotating chuck, exposing the employee to a caught-in hazard. f. On or about January 9, 2017, in the maintenance shop, an employee operated a Cutmaster lathe, Model YCL-3280, that did not have a guard to prevent contact with the rotating chuck, exposing the employee to a caught-in hazard. g. On or about February 7, 2017, in the maintenance shop, an employee operated a Cincinnati Bickford drill press that did not have a guard to prevent contact with the rotating chuck, exposing the employee to a caught-in hazard. h. On or about January 18, 2017, in the maintenance shop, an employee operated an Ikeda radial arm drill, type RM-1175, that did not have a guard to prevent contact with the rotating chuck, exposing the employee to a caught-in hazard. i. On or about February 14, 2017, in the mold repair shop, an employee operated a Feng Hsing lathe, Sodel CL4885G, that did not have a guard to prevent contact with the rotating chuck, exposing the employee to a caught-in hazard. j. On or about February 7, 2017, located in the mold repair department, employees operated a King Mill from King Machine Tool Division, Serial Number 3291 that did not have guarding to prevent employee contact with the rotating chucks, exposing the employees to a caught-in/struck-by hazard. k. On or about February 7, 2017, located in the mold repair shop, employees operated a King Mill from King Machine Tool Division, serial number 3263, that did not have guarding to prevent employee contact with the rotating chucks, exposing the employees to a caught-in/struck-by hazard. l. On or about February 7, 2017, located at the Kuka, KR 700 PA robot, employees inspected stenciling on wheels while being undetected by the robot due to inadequately installed light curtains, thereby exposing the employees to struck-by and/or crushing injuries.
