KC Midstream Solutions, LLC, 101 Queen Creek Rd, Endeavor, PA - On or about January, 11, 2017, and at times prior thereto: employees were potentially exposed to fire and/or explosion hazards in that, the employer had not established and implemented written procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affected the covered process at the Endeavor Natural Gas Processing Plant.
