This was most recently documented on January 4, 2017 at 1120 North 18th Street Omaha, Nebraska. The employer is failing to ensure that openings in 4x4 electrical boxes are effectively closed in the following but not limited to areas. a. Trim Cap behind North Wall the 4x4 electrical box knockouts were removed creating an opening in the box. b. Letter Shop behind the West Workbench the 4x4 electrical box knockouts were removed creating an opening in the box.
